Analysis of pregnancy outcomes before and after ERA in identical patients with RIF

Abstract

Objective: The aim of this study is to clarify the efficacy of ERA leading to personalization of the day of embryo transfer in RIF patients.
Methods: A retrospective study was performed for 94 patients with RIF who underwent ERA between July 2015 and December 2019. Pregnancy outcomes in a previous vitrified-warmed blastocyst transfer (previous VBT) and a personalized vitrified-warmed blastocyst transfer (pVBT) in identical patients were compared. The details of each pVBT were further analyzed between patients in a non-displaced group, which indicated “receptive” cases in ERA results and those who were in the displaced group, which indicated “non-receptive” cases.
Results: When the pregnancy rate, both per patient and per transfer cycle, of previous VBT and pVBT were compared, a significant increase in pVBT was observed between the two methods. The pregnancy rates, implantation rates, and clinical pregnancy rates of the first pVBT were significantly higher in the displaced group than the non-displaced group. The cumulative ongoing pregnancy rate of the displaced group tended to be higher compared to that of the non-displaced group in the first pVBT.
Conclusions: Our study demonstrates that pVBT guided by ERA tests may improve pregnancy outcomes in RIF patients whose WOI is displaced.

Biography

A graduate of Jikei University School of Medicine, he has trained extensively throughout Jikei University Hospital, Keiai Reproductive & Endosurgical Clinic, and Kyono ART Clinic Takanawa. He is currently on a staff at Kyono ART Clinic Takanawa and is also engaged in research on reproductive endocrinology as a graduate student at Jikei University School of Medicine. He is Board Certified Fellow of Japan Society of Obstetrics and Gynecology, and Japan Society for Reproductive Medicine.

Personalized embryo transfer improving clinical pregnancy rate in infertile women with RIF by EndomeTRIO testing

Abstract

Implantation is a complex process that requires synchronous and cross-talk between a healthy embryo and a receptive and microbiome composition endometrium. The aim of this study was to compare clinical outcomes in patients of recurrent implantation failure (RIF), who had embryo transfer following the EndomeTRIO testing and a personalized embryo transfers. The study was approved by the Institutional Review Board of Chung Shan Medical University Hospital, Taichung, Taiwan (CS-18191). This study included 93 infertility patients who have been advised ERA test and/or admitted to Lee Women’s Hospital, between November 2018 and May 2021. For a mock cycle prior their frozen blastocyst transfer cycle that included an endometrial biopsy for the EndomeTRIO testing. The clinical pregnancy rate of RIF patients with pET after non- receptive ERA was higher than the receptive ERA group (71.2% vs 51.2%). The clinical pregnancy rate after treating abnormal Endometrial Microbiome Metagenomic Analysis group was 80% (16/20). Performing the EndomeTRIO testing test increased the clinical pregnancy rate in RIF patients who were non-receptive implantation or exiting abnormal Microbiome endometrium. Further large prospective studies are needed to elucidate the role of EndomeTRIO testing testing in patients with recurrent implantation failure.

Biography

Dr Chun-I Lee was awarded an MD in 2008 from the National Defense Medical Center and completed his clinical training in reproductive medicine in 2016 in National Taiwan University College Medicine and his PhD from the Institute of Medicine, Chung Shan Medical University. Dr. Chun-I Lee professes on the Endocrine and Infertility medicine and Obstetrics and Gynecology. He is the CEO in Lee Women’s Hospital. He has worked with infertile couples for over 5 years and has achieved lots of healthy babies after treatment of couples by IVF. He has performed and published many studies on clinical reproductive medicine.

Whether ERA can effectively improve embryo implantation and clinical pregnancy rate in patients with RIF has been controversial. Through retrospective analysis, RIF patients were divided into experimental group (era-RIF) and control group (RIF). The implantation rate and clinical pregnancy rate after embryo transfer between the two groups were compared to determine whether ERA detection is effective for RIF patients.

Abstract

Many researchers have reported that asynchronous coordination between embryonic development and endometrial receptivity is the crucial cause of implantation failure, and the importance of finding the optimal and personalized endometrial receptivity time to improve implantation is emphasized.
In addition, research and the application of molecular evaluation are gradually developing to find means of a healthier pregnancy by checking the status of the endometrium in various aspects, such as microbiomes.
In this presentation, the endometria of patients with failed implantation were diagnosed using endometrial receptivity array (ERA) and histological assessment and analyzed according to clinical values such as biopsy time, age, number of implantation failures, and endometrial thickness. On that basis, we would like to share our recent experience with the personalized embryo transfer (ET) of blastocysts and embryos at the cleavage stage based on molecular diagnosis.

Recurrent implantation failures (RIF) are challenging all IVF teams and remain devastating for patients with infertility. Guidelines to perform exams have been provided after twice or three times of embryo transfers without pregnancy. However, a large part of these failures remains unexplained.
In this study, we present our strategy of a thorough investigation to understand the origin of these failures. Our results demonstrate that an individualized and multidisciplinary strategy should be adopted to improve implantation rate and live birth rate for couples with RIF.

Biography

Dr. Chien has more than 10 years of experience in Obstetrics and Gynecology, and specialized in Assisted Reproductive Technology Field. Dr. Chien graduated from University of Medical Sciences of Havana for his MD and then post-graduated with Excellent Degree.
Dr. Chien is now working mainly in the treatment for infertility couple, from basic techniques such as: control of ovarian stimulation, perform egg retrieval, Embryo transfer… to advanced techniques: finding and treating reasons of infertility, performing basic and advanced techniques in Obstetrics and Gynecology.
He is also one of the pioneers in using Endometrial Receptivity Analysis in Vietnam and has succeed in helping his patients delivering healthy babies.

Recurrent implantation failure is the most frustrating and distressing situation for patients and their treating doctor alike. This presentation will present a few cases where assessing the endometrial receptivity of the endometrium has been performed, and will discuss the indication for this invasive but often illuminating assessment.

Biography

Professor Roger Hart is sub-specialist in reproductive endocrinology and infertility (CREI), he is the Medical Director of Fertility Specialists of Western Australia and Professor of Reproductive Medicine, Deputy Head of the Division of Obstetrics and Gynaecology at University of Western Australia, and the lead clinician for the public fertility service of Western Australia. He has active research interests in the early life origins of male and female reproductive disorder, polycystic ovary syndrome, the longer-term outcomes after IVF treatment, and all aspects of infertility management, and has a particular interest in fertility preservation for patients with a cancer diagnosis. He has published over 200 chapters and research papers in the field of reproductive medicine.
